A few scheduling FAQ's:
- WHAT'S A FREE STRATEGY SESSION? We'll talk for free for 15 minutes to develop a strategy to help you meet your goals. In the process, we'll determine together if EVOLVE Flagstaff is right for you. Either way, you'll leave the call with a clearer path forward. We offer this service to help clear up the confusion and uncertainty that is so often part of healthcare. However, you are not required to schedule this call prior to your initial evaluation.
- DO YOU ACCEPT INSURANCE? We are out-of-network with all insurance companies, and charge a flat fee at the time of service (the amount is listed when you select an appointment type). We believe that this allows us to provide you with better care. We do accept Health Savings Account credit/debit cards, and can provide a detailed receipt with the billing codes you need to submit an out-of-network claim.
- DO I NEED A REFERRAL? You do not need physician's referral prior to your initial appointment.
- ANYTHING ELSE? You'll be emailed a short intake form after your schedule your first appointment through a secure, HIPAA compliant online system. Please fill this out prior to your appointment. If you fill out the forms, there's no need to arrive early.
