Please use the scheduling widget to select the appropriate appointment and a day and time that works for you. If nothing fits your schedule, contact us and we'll figure something out.
A few scheduling FAQ's:
DO YOU ACCEPT INSURANCE? We are out-of-network with all insurance companies, and charge a flat fee at the time of service (the amount is listed when you select an appointment type). We believe that this allows us to provide you with better care. We do accept Health Savings Account credit/debit cards, and can provide a detailed receipt with the billing codes you need to submit an out-of-network claim.
DO I NEED A REFERRAL? You do not need physician's referral prior to your initial appointment.
ANYTHING ELSE? You'll be emailed a short intake form after your schedule your first appointment through a secure, HIPAA compliant online system. Please fill this out prior to your appointment. If you fill out the forms, there's no need to arrive early.
